Blaine founded Rooted in Care with the dedicated mission of providing psychiatric services that feel steady, intentional, and clinically guided. Her work focuses on diagnostic assessment, medication decisions, and ongoing review for patients who need a structured approach to care.

Blaine Guelde-Patel

Integrative Nurse Practitioner

Blaine Guelde-Patel is an integrative nurse practitioner with more than 20 years of experience in healthcare. Her approach brings together medication management, non-medication techniques, and whole-person wellness to provide individualized care that considers each patient’s physical, emotional, and mental well-being.
Blaine began her Nurse Practitioner career with the Acute Pain Management Service at the University of Maryland’s R. Adams Cowley Shock Trauma Center, where she supported patients using both medication and non-medication approaches. In 2016, she transitioned into integrative medicine at the University of Maryland’s Center for Integrative Medicine in Baltimore, working across clinical care, research, education, and leadership. During this time, she trained in a variety of mind-body techniques, including Reiki, mindfulness, yoga nidra, and expressive arts.

Blaine earned her bachelor’s degree from Florida Hospital College of Health Sciences, now AdventHealth University. She received her Adult-Gerontology Acute Care Nurse Practitioner degree from Virginia Commonwealth University in 2013 and later completed her Psychiatric Mental Health Nurse Practitioner post-graduate certificate at the University of Cincinnati in 2019. While much of her clinical work involves medication management, she is passionate about incorporating non-medication strategies and whole-person wellness into treatment whenever appropriate. Her goal is to guide patients through their healing journey while being a supportive and positive presence in their lives.
Blaine continues to pursue educational opportunities that expand her knowledge and ability to care for her patients. Outside of her work, she enjoys spending time with family and friends and being a dog mom to her three dachshunds, Maddie, Bunnie, and Luna Belle.

Nicole M. Garcia, MSN, APRN, PMHNP, ANP

Psychiatric Mental Health Nurse Practitioner

Nicole M. Garcia is a board-certified Psychiatric Mental Health Nurse Practitioner with more than 20 years of experience in healthcare. Throughout her career, she has witnessed the importance of compassionate care, advocacy, and creating a space where individuals feel genuinely heard and understood. Her passion for helping others navigate mental health challenges led her to become a Psychiatric Mental Health Nurse Practitioner, where she strives to be a source of support, hope, and strength for her patients. Nicole offers in-person and telehealth visits and is currently accepting new clients. She provides comprehensive psychiatric evaluations, personalized medication management, and therapy for individuals experiencing depression, anxiety, bipolar and mood disorders, ADHD, PTSD, and other mental health concerns. She also supports patients navigating significant life transitions, including loss, college life, retirement, and periods of feeling overwhelmed or disconnected from themselves.

Her approach to care is integrative, combining psychotropic medication management with therapeutic strategies to help patients achieve greater balance and improve their quality of life. She believes treatment should address the whole person rather than symptoms alone and should be tailored to each individual’s needs, experiences, and goals. Nicole’s mission is to provide holistic, client-centered, evidence-based care while treating each client with the same compassion and consideration she would offer her own family. She creates a supportive, respectful, and nonjudgmental space where patients can better understand what they are going through, recognize their strengths, and work toward meaningful, lasting change.
